This participant assessment form was used to collect feedback from people attending Future Orchards® Orchard Walk activities in July 2015. It is a practical evaluation tool rather than a technical production resource.

The form supports structured feedback on the usefulness of an event and provides a record of how extension activities were assessed by participants. For organisations planning grower events, field days or training sessions, it also provides a historical example of the feedback mechanisms used within Future Orchards®.

The document can be downloaded and adapted as a reference for event evaluation, while recognising that current projects may require different reporting questions, consent wording or program branding.
